Community Health Donation Program

In keeping with the mission of Mille Lacs Area Health Foundation to support the diverse and changing health care needs of the Mille Las area, the Board of Directors established a donation program. Donations are made periodically from the general fund account of the Foundation to assist local non-profit agencies in addressing significant regional health and wellness issues.  

A sub-committee of the Board of Directors of the Foundation a cross-section of the community has established the guidelines and criteria for the donation program. The committee will consider programs that address any health-related issues, although priority will be given to projects developed by departments of Mille Lacs Health System that benefit the general community, youth, and family-related health problems, and prevention projects.


APPLICATIONS WILL BE REVIEWED THREE TIMES A YEAR 

AND ARE DUE TO THE FOUNDATION OFFICE ON APRIL 1ST,  AUGUST 1ST, AND DECEMBER 1ST. 

Awarded DONATIONS will be distributed in May, September and January.

Funding Considerations:

  • Reliability, past record, the mission of the organization

  • Financial feasibility of the project

  • Priority may be given for an organization pursuing multi-funding sources

  • Organizations may apply for up to $2,000 annually

  • Partial funding of a project may be considered

  • It is preferred that donation dollars are targeted for a specific project versus general operating funding

Donations will not be given for: capital campaigns, annual fund-drives, lobbying activities, past operating deficits, religious activities, athletic events, political parties or candidates, endowment funds, travel costs, seed money/start-up costs for a new organization, tickets or tables at fundraising events, charter/private schools, second or third party requests, advertising costs, operational support (there must be a specific intention), underwriting of any kind, exclusionary membership organizations, non 501(c)3 organizations, nonprofits outside a 50-mile radius of Onamia.

After funding is received, a thank you with an explanation of event results is expected within two months of the close of the event.

    your 501(c)3 tax ID form/ W9

Make sure the email with supporting documents has the subject ‘DONATION REQUESTS’.
